When to Use
- Use Z085 when performing debridement on two or more distinct wounds or ulcers that specifically involve deep tissue structures like tendon, ligament, bursa, or bone.
- Select Z085 over Z084 when the clinical documentation confirms the involvement of at least two separate sites reaching the specified deep structures, rather than a single site.
Common Pitfalls
- Billing Z085 alongside other surgical procedures on the same site is generally rejected unless you document specific complications that necessitate care beyond standard post-operative management.
- Failing to explicitly document the depth of the debridement (e.g., reaching bone or tendon) for each of the two or more lesions will lead to audit recovery, as the code definition requires this depth for all included sites.
- Billing Z085 in conjunction with restricted codes like Z154 or Z175 will result in automatic claim rejection due to the mutually exclusive nature of these wound care services.
Billing Tips
- If performing this procedure in your office, always append E542 to the claim to capture the additional fee for non-hospital settings.
- Ensure your operative note clearly lists each wound separately and describes the specific deep tissue involvement for each, as the Ministry requires proof of the 'two or more' threshold.

Documentation supporting the debridement of each separate lesion for which a claim is made is found in the medical record.
Wound dressing and wound and debridement services are not payable in addition to any surgical procedure unless complications require such care in excess of the usual post-operative care.
