Debridement of a single wound or ulcer extending into deep structures such as tendon, ligament, bursa, and/or bone. Payment Rules: - The physician must personally perform a minimum of 10 minutes of debridement. - Suture of laceration codes (Z154, Z175, Z176, Z177, Z179, Z190, Z191, Z192) and complex laceration repair codes (Z187, Z188, Z189) are not eligible for payment with this service. - The service includes the application of any necessary dressing. - Wound debridement services are not payable in addition to any other surgical procedure unless complications require such care in excess of the usual post-operative care. Add-on Premium: - E542 is payable when this service is performed outside a hospital.
When to Use
- Use Z084 when you perform at least 10 minutes of active debridement on a single wound that involves exposure of deep structures such as bone, tendon, ligament, or bursa.
- Use Z084 for the surgical debridement of a chronic diabetic foot ulcer or pressure injury where necrotic tissue removal extends into the underlying fascia or bone.
Common Pitfalls
- Billing Z084 alongside any laceration repair code (e.g., Z154, Z175, Z187) will result in an automatic rejection as these services are mutually exclusive.
- Failing to document the specific start and end times of the debridement process is a primary cause for audit recovery, as the 10-minute minimum is a strict requirement.
- Attempting to bill Z084 for routine wound dressing changes or superficial debridement that does not reach the specified deep structures.
Billing Tips
- If you perform the procedure in your office rather than a hospital, ensure you append the E542 premium to receive the additional $11.55 payment.
- Clearly document the depth of the debridement and the specific structures encountered (e.g., 'debridement of necrotic tissue down to exposed calcaneal bone') to substantiate the use of Z084 over lower-level codes.
