Laser angle surgery is an ocular surgical procedure. The fee for this service encompasses all common elements of an insured service, including but not limited to, making arrangements for appointments, obtaining and reviewing patient information, maintaining medical records, and providing prescriptions as outlined in the General Preamble (-). The anaesthesia fee is calculated based on 7 base units plus time units, as specified on page of the Schedule of Benefits.
When to Use
- Use E134 for endoscopic cyclophotocoagulation (ECP) performed to reduce intraocular pressure in patients with refractory glaucoma.
- Use E134 for trans-scleral cyclophotocoagulation (TSCPC) when laser energy is applied to the ciliary body to decrease aqueous humor production.
- Use E134 for laser angle surgery procedures specifically targeting the trabecular meshwork or ciliary body, distinguishing it from standard laser trabeculoplasty (E131).
Common Pitfalls
- Do not bill E134 in conjunction with other glaucoma laser procedures like E131 (laser trabeculoplasty) on the same eye during the same session, as this is considered unbundling.
- Avoid billing E134 for routine laser iridotomy (E130); E134 is strictly for cyclodestructive or angle-specific surgical procedures.
- Failure to document the specific laser modality (endoscopic vs. trans-scleral) can lead to audit scrutiny if the claim is flagged for clinical review.
Billing Tips
- Ensure the appropriate age-based premium is applied if the patient is under 16 years of age, as these are significant percentage increases to the base fee.
- If the procedure is performed on an emergency basis after hours, ensure the correct suffix and premium (E409A or E410A) are applied to the surgeon's fee to capture the 50-75% increase.
