E148
E148 – Vitrectomy, complete and by posterior approach, with vitreous exchange
OHIP Surgical Assists Code — OCULAR SURGICAL PROCEDURES · Schedule of Benefits
Vitrectomy by infusion suction cutter technique. This is a surgical procedure and as such includes a pre-anaesthetic evaluation, the anaesthetic procedure itself, and post-anaesthetic follow-up. It also includes the common elements of insured services as outlined in the Schedule of Benefits (-).
When to Use
- Use E148 for a complete pars plana vitrectomy involving the removal of vitreous gel and the performance of a vitreous exchange using air, gas, or silicone oil.
- Select E148 when performing a vitrectomy for complex retinal detachments or vitreous hemorrhage where the infusion suction cutter technique is required, as opposed to simpler procedures like E142 (vitrectomy for vitreous opacity).
Common Pitfalls
- Billing E148 with E940 is a common rejection, as E940 (removal of silicone oil) is considered an included component of the vitrectomy procedure.
- Failure to append the E936 add-on code when a vitreous exchange is performed results in lost revenue, as E936 is specifically designed to capture the additional work of the exchange.
Billing Tips
- Always verify if the procedure qualifies for age-based premiums (e.g., AGE_PREMIUM_5Y_16Y) or after-hours premiums (E409/E410) to maximize the claim value, as E148 is a high-value surgical service.
- Ensure the operative report explicitly details the use of the infusion suction cutter technique to support the use of E148 over lower-valued ocular surgical codes.
