When to Use
- Use E776 when performing a brush biopsy of the ureter or renal pelvis during a diagnostic cystoscopy (Z606 or Z607) to investigate upper tract urothelial carcinoma.
- Use E776 for a transurethral bladder biopsy when the procedure is limited to biopsy only and does not involve the formal excision or resection of a bladder tumor.
Common Pitfalls
- Billing E776 alongside Z632, Z633, or Z634 is a common rejection; these excision codes are comprehensive and already include the biopsy component.
- Attempting to bill E776 as a standalone procedure will result in a rejection, as it is strictly an add-on code that requires a primary cystoscopy code (Z606/Z607) on the same claim.
Billing Tips
- Ensure the primary procedure (Z606 or Z607) is billed on the same claim to avoid the automatic rejection of the E776 add-on fee.
- If performing both a ureteral brush biopsy and a bladder biopsy during the same session, E776 is only payable once as it covers both anatomical sites.
