E668 – Cannulation of minor papilla

This code represents an additional fee for the cannulation of the minor papilla when performed during an Endoscopic Retrograde Cholangiopancreatography (ERCP) procedure. As per the note on page of the Schedule, this code is only payable in conjunction with a primary ERCP service, such as Z561, Z558, or Z760.
When to Use
- Use E668 when you successfully cannulate the minor papilla to perform a pancreatic ductogram or therapeutic intervention during an ERCP.
- Use this code specifically when the minor papilla is the target for access in cases of pancreas divisum, where the major papilla does not provide adequate drainage.
Common Pitfalls
- Billing E668 without an associated primary ERCP code like Z561, Z558, or Z760 will result in an automatic rejection.
- Attempting to bill E668 for cannulation of the major papilla is incorrect; it is strictly reserved for the minor papilla.
Billing Tips
- Ensure E668 is submitted on the same claim as the primary ERCP procedure code to satisfy the requirement for concurrent billing.
