This is an add-on code representing an oesophagoscopy-gastroscopy, which may include duodenoscopy, performed concurrently with an Endoscopic Retrograde Cholangiopancreatography (ERCP). As per the note on , this service is only payable in addition to one of the primary ERCP procedures: Z561 (ERCP with cannulation), Z558 (ERCP with sphincterotomy and/or stone removal), or Z760 (ERCP through gastrojejunostomy).
When to Use
- Use E669 when you perform a diagnostic esophagogastroduodenoscopy (EGD) immediately preceding an ERCP procedure (Z561, Z558, or Z760) to visualize the anatomy or clear the field.
- Use this code when the EGD is a distinct procedural component required to facilitate the subsequent ERCP, provided the ERCP is the primary procedure billed.
Common Pitfalls
- Billing E669 as a standalone procedure will result in an automatic rejection, as it is strictly an add-on code that requires a primary ERCP code on the same claim.
- Attempting to bill E669 alongside a standard diagnostic gastroscopy (such as E662) for the same patient encounter is considered double-dipping and will lead to audit recovery.
