Z760 – ERCP through gastrojejunostomy following previous Billroth II
OHIP Psychiatric Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
Endoscopic retrograde cholangiopancreatography (ERCP) through gastrojejunostomy following previous Billroth II procedure. As a surgical procedure, it includes all common and specific elements of surgical services, such as pre-operative assessment, the procedure itself, and post-operative care, as defined in the Schedule of Benefits. Unless otherwise specified, there is no additional fee payable for a cholangiogram performed during the surgery.
When to Use
- Use Z760 specifically for ERCP procedures performed in patients with altered anatomy due to a prior Billroth II gastrojejunostomy.
- Do not use Z561 or Z558 for these cases, as Z760 accounts for the increased technical complexity of navigating the gastrojejunostomy.
Common Pitfalls
- Billing Z760 with a standard ERCP code (Z561 or Z558) will result in a rejection for duplicate service or incorrect code selection.
- Failing to document the specific Billroth II anatomy in the procedure note can lead to audit recovery, as the higher fee for Z760 is predicated on this specific anatomical challenge.
Billing Tips
- Always append E669 if an oesophagoscopy-gastroscopy is performed to access the gastrojejunostomy, as this is a separately payable add-on to the base Z760 fee.
- If you perform multiple stent insertions, bill E680 for the first stent and use E681 for subsequent stents up to the maximum of three additional units.
