This code represents the performance of biliary tract manometry as an additional procedure during an Endoscopic Retrograde Cholangiopancreatography (ERCP). It is intended to be billed as an add-on to a primary ERCP service. As per the note on (), E666 is payable with Z561 (ERCP with cannulation of common bile duct and/or pancreatic duct), Z558 (ERCP including sphincterotomy), or Z760 (ERCP through gastrojejunostomy).
When to Use
- Use E666 when performing sphincter of Oddi manometry during an ERCP to assess for sphincter of Oddi dysfunction in patients with unexplained biliary-type pain.
- Apply this code when manometric pressure measurements are obtained as a diagnostic adjunct during a primary procedure coded as Z561, Z558, or Z760.
Common Pitfalls
- Billing E666 as a standalone procedure will result in an automatic rejection, as it is strictly an add-on code that requires a primary ERCP service.
- Attempting to bill E666 with ERCP codes outside of the approved list (Z561, Z558, Z760) will trigger a claim rejection.
- Failure to document the manometry findings in the procedure report is a significant audit risk, as the code specifically covers the performance of the manometry, not just the intent.
Billing Tips
- Ensure the primary ERCP code (Z561, Z558, or Z760) is listed first on the claim to satisfy the add-on requirement for E666.
