Chronic, slow continuous ultrafiltration (SCUF) is a diagnostic and therapeutic procedure under the 'DIALYSIS' subcategory. It is the chronic management component of this therapy, distinct from the initial and acute phase covered by G090. As per the note on page , services covered by team benefits are included, but this does not cover the preliminary investigation of the case.
All insured services must be documented in appropriate records. The Act requires that the record establish that:
1. an insured service was provided;
2. the service for which the account is submitted is the service that was rendered; and
3. the service was medically necessary.
