Insertion of a secondary intraocular lens. The surgeon's fee (<suffix>A</suffix>) is a flat rate and includes retrobulbar injection when administered by the surgeon. Assistant services (<suffix>B</suffix>) and anaesthesia services (<suffix>C</suffix>) are also payable for this procedure, with their fees calculated based on units as described in the payment rules.
When to Use
- Use E146 specifically for the secondary insertion of an intraocular lens in an aphakic eye that has already undergone a previous cataract procedure.
- Use this code for revisions or secondary procedures where an artificial lens was not placed during the initial surgery or requires replacement.
Common Pitfalls
- Do not bill E146 for primary cataract surgery; it is strictly restricted to secondary lens insertions or revisions.
- Anaesthesia services (E146C) are limited to 6 basic units; billing for additional time units or extra units listed on GP97 will result in rejection.
- E146 is mutually exclusive with E140, E141, and E144; attempting to bill these on the same claim will trigger a rejection.
Billing Tips
- Ensure the retrobulbar injection is not billed separately, as the fee for E146A is all-inclusive when performed by the surgeon.
- When claiming anaesthesia, use E146C but note that payment is governed by the 6-unit cap established for E023C.

E003C is not payable for anaesthesia services rendered for E138, E139, E140, E141, E144, E145 or E146. 3. E145 and E146 are only eligible for payment for revisions to previously implanted artificial intraocular lens. 5. E146 is not eligible for payment with E140, E141 or E144.
