Electrophysiologic pacing, mapping and ablation - ventricular pacing and mapping. Includes percutaneous access, insertion of catheters and electrodes, electrocardiograms, intracardiac echocardiograms and image guidance when rendered. Z423 (advanced nonfluoroscopic computerized mapping and navigation system and/or procedure duration >4 hours) is only eligible for payment with G176 or G177. G177 is listed as a major invasive procedure for after-hours procedure premiums ().
