Per the note on , the ovarian excision codes include payment for unilateral or bilateral services, except that E090 may be added to S745 for removal of the contralateral ovary with moderate or severe endometriosis. S745 is not eligible for payment when rendered with S757, S816, S758, S759, S710, S763 or S762 ().
