Surgical excision of one or both ovaries and/or ovarian cysts, performed by laparotomy. The fee for this service includes payment for unilateral or bilateral procedures. Per a note on , E090 may be claimed in addition for the removal of the contralateral ovary if it has moderate or severe endometriosis.
When to Use
- Use S745 for the surgical excision of one or both ovaries or ovarian cysts via laparotomy.
- Use S745 in conjunction with E090 when performing a bilateral oophorectomy specifically where the contralateral ovary requires removal due to moderate or severe endometriosis.
Common Pitfalls
- Attempting to claim S745 twice for a bilateral procedure; the fee is inclusive of both unilateral and bilateral services unless E090 criteria are met.
- Billing E090 for a bilateral oophorectomy performed for reasons other than moderate or severe endometriosis, such as prophylactic removal or simple cysts, which will result in a rejection.
- Confusing S745 with laparoscopic ovarian procedures; S745 is strictly for laparotomy (open) approaches.
