- Select S763 when performing a radical hysterectomy for cervical cancer where the extent of resection includes the upper vagina and parametrium.

- Do not bill S763 in conjunction with S760 (total hysterectomy) or S722 (radical vulvectomy), as these are explicitly restricted and will result in claim rejection.
