G414 – Routine EEG - technical component

A routine EEG consists of at least a twenty minute recording with referential and bipolar montages and at least eight channels (except in neonates). Hyperventilation and photic stimulation should be done in all cases where clinically possible.
When to Use
- Use G414 when billing for the technical component of a standard 20-minute EEG that includes both hyperventilation and photic stimulation.
- Use G414 when a sleep-deprived or video-EEG study fails to meet the specific criteria for higher-level codes, requiring a downgrade to the routine EEG fee.
Common Pitfalls
- Billing G414 alongside G541 will result in an automatic rejection as they are mutually exclusive.
- Submitting G414 on the same day as any sleep study codes (e.g., J898, J899, J990) is prohibited and will trigger a claim rejection.
- Failing to account for the 13.9% reduction in fee when the technical component is performed within a hospital setting.
Billing Tips
- Ensure your documentation explicitly notes the performance of hyperventilation and photic stimulation, as these are mandatory requirements for the technical component claim.
- If performing the service in a hospital, ensure your billing software is configured to apply the 86.10% payment adjustment to the G414 fee.

The physician submitting a claim for the technical component is responsible for the complete quality assurance process for all elements of the technical component of the service, including data acquisition, reporting, and record keeping. The physician must be able to demonstrate the above upon request by the MOH.
The physician maintains documentation that describes the process by which the physician monitors quality assurance in accordance with professional standards.
The amount payable for a sleep-deprived/induced EEG, or an EEG with time-locked video recording that does not meet the above requirements will be reduced to that for a routine EEG fees (i.e. G414 and G415/G418).
G414 is not eligible for payment with G541.
Only one of G415, G418, G543 or G496 are eligible for payment per day.
EEG services (i.e. G414, G415, G418, G496, G541, G543, G540, G545, G542, G546, G554, G555, or G544) are not eligible for payment with any overnight or daytime sleep study (i.e. J898, J899, J990, J896, J897, J895, J890, J889, J893 or J894).
