An EEG with time-locked video recording must include all of the following: - a. at least 30 minutes of EEG recording time; - b. a minimum of 16 channels of EEG; - c. recordings of at least two physiological parameters; and - d. a time-locked video recording. G496 represents the professional component for this service.
When to Use
- Use G496 when performing a diagnostic EEG specifically requiring time-locked video for the evaluation of paroxysmal events or suspected seizures.
- Use this code when the study meets the strict 30-minute minimum duration and 16-channel requirement, distinguishing it from a standard routine EEG (G414).
Common Pitfalls
- Billing G496 alongside any sleep study codes (e.g., J898, J899) will trigger an automatic rejection due to explicit billing restrictions.
- Failing to document the two required physiological parameters in addition to the EEG will result in a payment reduction to the lower-valued routine EEG fee (G414/G415/G418).
- Submitting G496 on the same day as G415, G418, or G543 is prohibited, as only one of these codes is eligible for payment per patient per day.
Billing Tips
- Ensure your report explicitly lists the 16 channels used and the two physiological parameters recorded to defend against audit-based payment adjustments.
- If the study duration falls below 30 minutes, do not bill G496; instead, use G414 to avoid a rejection or manual adjustment.

The amount payable for a sleep-deprived/induced EEG, or an EEG with time-locked video recording that does not meet the above requirements will be reduced to that for a routine EEG fees (i.e. G414 and G415/G418).
Only one of G415, G418, G543 or G496 are eligible for payment per day.
EEG services (i.e. G414, G415, G418, G496, G541, G543, G540, G545, G542, G546, G554, G555, or G544) are not eligible for payment with any overnight or daytime sleep study (i.e. J898, J899, J990, J896, J897, J895, J890, J889, J893 or J894).
